Queues of 10km long have formed on the M7 in the wake of a seven-vehicle crash between three semi-trailers and two utes that police are investigating.
Around 3pm, ambulance officers assessed seven people following the crash on the motorway at Oakhurst.
Police are investigating and said the believed two southbound trucks crashed before two utes travelling behind them collided while attempting to avoid the incident.
They said a third truck towing a trailer jack-knifed as a result of the crash.
Two people were taken to Westmead Hospital and another two were taken to Blacktown Hospital, with three of the injured complaining of chest pains and one complaining of neck and knee pain.
They are all in stable conditions.
Meanwhile, a diversion is in place on the M7 between Richmond Road and Woodstock Road, with motorists encouraged to avoid the area if they can.
While the motorway has reopened southbound, traffic remains extremely heavy in both directions.
Police advise drivers to avoid that part of the highway.
